The Freedom of Information Act 1982 (Vic) (FOI Act) gives persons the general right, subject to specified exceptions, to access information held by agencies, departments and authorities of the Victorian Government. Alpine Health is an agency under the FOI Act.
Can I access my own health information?
You have the right to request access to your medical record and personal information held by Alpine Health, in accordance with the Freedom of Information Act 1982 (Vic) (FOI Act) and the Health Records Act 2001 (Vic). It is possible to obtain copies of information, view information or request an amendment to information that is incomplete, incorrect, out of date or misleading.
How do I access my health information?
All requests for access to documents held by Alpine Health under the provisions of the FOI Act are to be made in writing.

Costs
The total cost of producing a record varies according to the type of request. These charges are set by government regulations. For further information please visit www.ovic.vic.gov.au
Fees and charges (as at 1 July 2024)
- Application fee: $32.60 (Indexed annually)
- Search fee: $22.20 (or part of an hour)
- Supervision: $5.00 per 15 mins
- Photocopying: 20c per A4 page
- Current postal charges also apply.
An application for financial hardship may be made but must be supported by concession card evidence such as a Health Care or Pension Card. If your application is accepted, you may be excused from paying some or all of the charges.

We may write to you again following our assessment of your application, to request a deposit, if the access charges are estimated to be in excess of $50.
