Notice is hereby given that the Alpine Aged Care Advocacy Service will cease operations effective 21 June 2024.
In line with the Victorian Health Privacy Principals (Principal 10 - Transfer or Closure of the Practice of a Health Service Provider) the Alpine Aged Care Advocacy Service (as the legal guardian) proposes to transfer the records of current and previous clients to Alpine Health for safe storage until such time as the records can lawfully be destroyed in accordance with Health Privacy Principle 4.
Should previous clients of the Alpine Aged Care Advocacy Service wish to have their record forwarded to their current health service provider, they are to complete an application, and make payment of $10 (registered post costs).

Records will only be sent to a current health service and applicants must provide the name and address of the health Service to whom the records are to be sent.
